I accept cash, check, or credit card payments at the time of service.
I am also an in-network provider for the following major medical insurance companies: Blue Cross Blue Shield, Aetna, UnitedHealthcare, and Cigna.
I will process your claims directly if you have acupuncture coverage from the above listed insurance providers. In order for me to accept your insurance as payment, the following conditions must be met:
You need to send me your insurance information in advance of your first appointment so I can check your benefits before you are accepted into the practice and arrive for the appointment
You need to bring your insurance card and a photo ID to your first appointment
At the time of your first appointment, you need to sign the agreement with my office to agree to pay any remaining funds that your insurance plan does not cover. This includes any deductible that must be met and any copay due at the time of service
If your insurance plan requires a referral or preauthorization, you need to make those arrangements prior to your first appointment
If you have acupuncture coverage from an insurance company other than the ones listed above, I will give you a super bill with the proper information needed to assist your claim to be processed to get partial or full retroactive reimbursements for the payments you have made. Please contact your insurance company to learn about your out-of-network acupuncture coverage.
In addition, most Health Savings Accounts (HSA), Flexible Spending Accounts (FSA), or pre-tax savings programs will allow you to claim acupuncture services and some cover the cost of Chinese Herbs.
